Urinary tract infection in long-term care setting.

Elderly are the group of individuals that easily to catch infections. Most of common infections such as: pneumonia, wound, urinary tract infection, etc. are the causes that make elderly become sick and even death in long-term care facilities. These infections are not complicate one to be treat. They are curable. However, it is hard to treat it right for elderly in long-term facilities in my opinion. It involved leadership, team and collaborate, management, financial, staffs training and patient teaching. These are the one needed to be change and improve in patient care to delivery good quality cares to patients.

Rationale for Selecting the Research Question (Type in box below)

At long-term care facility where I am working, I have seen a lot of patients got infected with the UTI mostly. The percent of infected people kept rising constantly each month on the reports. This type of infection can be preventable if we have enough well-educated staffs and patient teaching. In my per view, I would like to see the percent of infected people decrease, and patients get well recovery. For those individuals who still able fully function, we will provide constant patient teaching about signs and symptoms to report, hygiene to prevent infection or recurrent. For those individuals who unable function, we will have enough well-trained staffs to check on patients frequently at least 2 hours to help them change and perform hygiene to prevent infection. Also, staffs will learn how to prevent cross contamination.

I found these three articles by using CINAHL Complete at University of Arlington Library. These articles have topics related to improving the cares, staffs training, and education related to Urinary Tract Infection topic that I chose to represent. They are peer views and evident base practice articles which are within five years as required of this assignment.

As the number old people increasing, there is an increasing demand for health care staffs to take care of them. A lot of long-term care facilities recruited staffs with limited training and knowledge despite they have passed certificate to work in this medical field (Means, 2016). These staffs needed have more time in training and needed to attend any continuing education classes that provided at their workplace. They need sufficiency education to be able to recognize early signs and symptoms of any infection, especially the common one-UTI, to have patients get treat immediately. I chose this article because it points out the fact that the employments did not make mandatory training for staffs (Means, 2016). Thus, staffs will not take it serious and willing to attend to the training for obtaining new knowledges in work. Working in medical field require individual to be serious to learn, apply their knowledges and skill to take care of vulnerable population. Mostly, the long-term facilities are focusing on their profits, saving budgets, have poor managements and teamwork. These current situations are not only making patients to suffer but also cost a lot of money from the patients, government system for ineffective treatments.

There is a lot of research for UTI topic that related to catheter articles on the internet.  People used CAUTI as the guideline to exclude the non-catheter UTI. Not many sources be specific on non-catheter UTI for staffs to learn to prevent this infection. To prevent the non-catheter UTI, it just requires in generalize such as daily assessment, monitoring, intervention but they are not work highly effective (Sublett, 2016). The reason I chose this article because it shows the fact that there are not many resources on the non-catheter UTI comparing catheter related UTI base on this evidence base practice article.  It addressed to my research question because if there are not many researchers investigate this problem, then how can we start their theories, research, and study to build a guideline for others.

For effective staffs training and education, also patient teaching included, the long-term facilities need to create presentations the involved three sections. First section is identifying the risk factors such as patients who with comorbid diseases, physical or memory disability are the high-risk population that require addition cares than others who are healthy. Besides, teach staffs and patients to recognize any first sign and symptoms to report. Second section is focus on interventions included assessment, reporting, treating, and monitoring. Final section is focusing the physical routines of both staffs and patients to prevent UTI and recurrent (Freeman-Jobson, Rogers, & Ward-Smith, 2016)
