Daily activity description

what I need is a report containing a brief description of a therapy session with a client who has behavior problems. every session took place six day a week around 3 hrs each one. I’m going to attached and example and also her maladaptive behaviors in order to the activity described in the report better match with her. for any question don’t hesitate to contact me.
Note: I need 5 pages per months since I am delayed with 3 months so please do 3 separated documents, one per month. thanks
