Ms. H contacted the local food bank to say she had no food in her home. The food bank notified Adult Protective Services and asked for a wellness check for Ms. H. Upon entering the home, the social worker found that the patient was living in a small, run-down apartment without proper ventilation. The apartment was very cluttered and there was no food in the cupboard. The refrigerator had ½ can condensed milk that was 3 month past the consume-by date. Ms. H. had an unkempt appearance. She suffers from diabetes, hypertension and high cholesterol. Her diabetes has left her severely visually impaired and unable to deal with homemaking duties. Although nearly blind, she does manage to use public transportation and attends a congregate meal site when she is able. The social worker got the name of Ms. H’s doctor from her medication bottle and notified her of the situation. The doctor ordered a home health evaluation for disease management, diabetes teaching, and personal care attendant hours.
