He is actively having a psychotic episode that includes auditory hallucinations, where he says he hears voices in his head call him “Lord”. He has delusions of grandeur where he describes ruling the world, along with his band of followers that he calls “the death eaters”. He appears completely out of touch with reality, claiming that he can live forever.  Upon reviewing his medical record, you see that Mr. R was diagnosed with schizophrenia many years ago. His medication had been changed recently to Haldol 2mg PO BID. Apparently, Mr. R has been noncompliant with taking his medication because he failed to refill his Haldol last month. Discuss: 1. Explain what type antipsychotic Haldol is categorized as and how it works. 2. After somewhat calming him down, Mr. R was directly asked if he has been taking his medication. Mr. R says he would skip doses because he was afraid the medicine was causing him to physically change into someone else, because his “back, neck, and face would move involuntarily and repeatedly”. What might Mr. R have been experiencing and why? 3. Antipsychotic medications are available in many formulations. Conduct an online search on the difference in formulation between a regular IM injectable formulation and a depot IM injectable formulation. Compare and contrast these two formulations. 4. Using the tables in your textbook only, select a long acting medication specifically indicated to treat schizophrenia that MR. R may be switched to, that does not require him having to take daily oral maintenance doses. Include the full trade name and dosage of the medication you chose.
